Once completed, please print and fax this form to us at 602-371-0874. We will then change your account at TriTekElectronics.com to "tax exempt" status for all future sales.

If you would like to make taxable purchases in addition to non-taxable purchases, we recommend creating 2 onilne accounts.

If you are a contractor, installer, or reseller from outside of Arizona and we are shipping to your location outside of Arizona, you will be exempt from Arizona sales tax and there is no need to complete the Form 5000.
